Chronic Disease Management
Comprehensive Chronic Care
Not all medical conditions can be cured with medication or a procedure. Chronic diseases are long-term conditions that may progress slowly over time and require special care.
For patients with a chronic disease, Dr. Joyce Tatelman provides ongoing care and support to help manage your condition on a daily basis. The most common chronic conditions we see in our practice include:
- High blood pressure (hypertension)
- Type 2 diabetes
- Heart disease
- Congestive heart failure
- Arthritis
- Asthma
- Chronic obstructive pulmonary disease (COPD)
- Kidney disease
- Lipid disorders
- Thyroid disorders
The goals of chronic disease management are to minimize symptoms, improve quality of life, and prevent unnecessary hospitalizations.
To achieve these objectives, Dr. Tatelman creates a comprehensive care plan that includes your health issues and goals, other health care providers you may need, your medications, community services you may need, and other information pertaining to your care needs, as well as how your care will be coordinated.
She may also provide referrals to specialists as necessary, as well as follow up if you require hospitalization or are transferred from one health care setting to another. Dr. Tatelman will communicate with other providers so she can stay abreast of the care you receive and follow up appropriately.
While patients of any age can be affected by chronic disease, these problems tend to be more prevalent among seniors. In fact, many of our older patients have more than one chronic condition. Because these conditions typically involve multiple medications as part of the treatment plan, medication management is a critical component of chronic disease management.
With advanced training in geriatrics, Dr. Tatelman is well-versed in the side effects of medications and any negative interactions that may occur from taking multiple drugs. She can help you or your loved one understand these effects and develop a consistent medication management routine to improve compliance.
